What this consultation is about

As someone who has had a short term service over the last year we would appreciate your time in completing this questionnaire.

We have recently carried out several engagement meetings talking to stakeholders about our Adult Care Strategy and this survey is based on feedback from those meetings.

The top emerging theme from those meetings was concerning information and advice. The questions have been developed to give a greater weighting to that theme.

The information you provide in this survey will help us to understand what we get right and how we can improve our services for other people in the future.
